- Team Rules Form which must be signed by all swimmers. One form has room for four swimmers.

IMPORANT NOTE: Please disregard the information in the form about making check payable to Pacific Swimming. Include your USA registration fees in the same check as your Swim Team Dues. The team will forward your registration and fees to Pacific Swimming.
